Responsibilities and Duties:
Deliver supports in line with participant care plans and goals
Monitor wellbeing and report/document incidents or changes
Maintain professional boundaries and complete ShiftCare notes accurately
Requirements:
Passion for supporting people with disability
Experience in disability support (preferred)
Cert III or IV in Disability/Individual Support (or working towards)
Current First Aid & CPR
NDIS Worker Screening Check & Blue Card (or willing to obtain)
Driver’s licence & reliable vehicle (for community access roles)
